Preanalysis
- Whenever possible take blood culture before giving antibiotics.
- In some clinics, three bottles aerob/ anaerob are taken with 15 minutes spread in between.
- Bring the blood culture directly to the lab.
Analysis
- The blood will be put in an incubator system (for example Bact/Alert).
- This machine will make a lot of noise to attract medical technician when something is growing in the bottle. The machine detect products of biochemical reaction formed in the bottle.
- The bottle will be removed and Gram stain is performed.
General indication about the suspect can already be indicated.
- Culture is performed.
after 24 hours, the agar can be read and when something is growing there, identification can be performed using MALDI.
- Perform antibiogram.
Notes
- In pediatrics patients it might be difficult to obtain many bottles. It can be opted to culture only aerobically. When it is positive, the bottle can be culture anaerobically,
