Vacuum assisted closure and bacterial load

Vacuum assisted closure (VAC) is a more known name of negative pressure wound therapy (NPWT). Many of us have seen this during our internship surgery. It is a system consists of a foam covered by a plastic that is connected to a machine that creates negative pressure. It is believed that this system accelarate wound healing by removing bacteria in the wound. Yet, this evidence is based mainly on animal studies. So, we wanted to know about microbiological state of the foam itself. We sonicated the foams and culture the sonication fluid. We found that in average 10E5 CFU/ml bacteria in a foam. Also, changing the foam very 3 days did not bring the number of bacteria down.

We think, to reduce the number of bacteria in the foam, we should do more: change the foam more frequently or instill the foam with antiseptic.
